Scar Revisions

Becky writes, “I have been in remission from childhood cancer for 18 years but still have a horrible portacath scar on my breasts. I've been wanting to get this corrected for most of my life.”

Dr. Salzhauer: A porta-cath is like an IV that's put into a major vein in your chest when you have to get chemotherapy and can leave a bad scar, and especially when your eight years old, but now she's 22, the scar bothers her, she cant wear low tops and that sort of thing. The answer is yes, they can be revised and can be revised to be much smaller. They can't go away; however, we can put it in a way so that it's less conspicuous.

Dimples

Norea asks, “I want to know if you do dimples, I want to do it for both cheeks, I'm a girl, I'm 25, and what does it cost?”

Dr. Salzhauer: Yes we can do dimples, you can make dimples, and I've done it for men and women. It involves using a permanent suture that's threaded through your skin from a very inconspicuous area in your ear down to just the place you want the dimples in your cheeks, it comes back out and is tied down, and theoretically it lasts forever. The cost is really not that bad, it's in the $2,000-$3,000 range. It does involve some local anesthesia or sedation, and it takes about ½ hour to 45 minutes for the procedure.

Sun Damaged Skin

Tim: I have a lot of sun damage, its not blotchy like it used to be, but I'm 50 and have a lot of lines and kind of basset hound skin, and I live in the sun so 10 years ago I went in to ask if there's anything they could do, but I was told as long as I continued getting sun, there was no procedure I could do, other than micro-dermabrasion. I did that for a year, but I think even that damaged me because it stripped my skin down so bare, that the sun did even more damage, and started looking bad after it looked good for awhile. I wonder are there any procedures now that will allow someone to continue to get sun? I wear a hat and SPF 60 sunscreen. I don't want another procedure that strips my skin so bare that it damages my face more.

Dr. Salzhauer: Your concern is obviously of importance to many people in South Florida, particularly people who grow up here and bask in the sun their entire lives. By the time their in their 40s & 50s you do see significant damage. There is a topical medication called Retin-A which does clinically reverse the actual sun damage on a metabolic cellular level, and it's the only proven common popular cream that does it. Mid Face Lift

Rick: Do you do mid-face lifts or where you just do the eyes?

Dr. Salzhauer: It's a very popular operation actually. The mid-face is where a lot of people start to show aging earliest. Of course we do neck lifts and formal face lifts but the mid-face is around the eyes, basically from the eyes below to the mouth area and what happens is as the tissue of the cheek region starts to fall with gravity, you get those deep lines called nasal labial folds, they also change the shape of the face from looking nice, full, healthy and round to a more gaunt look. Pectoral Implants

Janelle: I was interested in pectoral implants for my boyfriend.

Dr. Salzhauer: The short answer is no, I do not recommend them in cases where people have even normal amounts of chest wall muscle. If your boyfriend has a serious deficiency involved in that area I might consider doing it. The reason is that pectoral implants have a fairly high complication rate; as you age, they tend to look more and more unnatural and often times have to be taken out.

Breast Augmentation

Cathy emails to ask, “What is sub glandular and sub muscular when talking about breast augmentation? She also says she wants to do her nose and breasts at the same time, and is this possible? Also, I am looking for a realistic gel, and does this leave a bigger scar?”

Dr. Salzhauer: Sub glandular versus sub muscular. When you talk about breast augmentation, there are two different planes the implants can be put into, one is on top of the muscle, the muscle we're talking about is the pectoralis muscle, the one built up in the chest, and if you put the implant underneath the muscle, you get the advantage of having an extra layer of tissue between the implant and the outside world, so it does tend to mask the implant a bit more and can feel more natural in many cases. Lip Enhancement

Nancy: I am interested in lip enhancement.

Dr. Salzhauer: Have you ever tried any injections at all of collagen or restalyn or anything?

Nancy: Yes, but I wanted something more permanent.

Dr. Salzhauer: A lot of people have been getting restalyn or juvederm injected into their lips for years. They are hypertonic acid based materials, they last from eight months to a year, give a nice natural result, but they do dissipate, your body absorbs them over time, and you're left with whatever thin lips you had. Feather Lift

Eleanor: I was calling to ask what the feather lift is and does it work?

Dr. Salzhauer: The feather lift has fallen out of favor among most plastic surgeons over the years. It is a suture with barbs along the sides of it. The idea being that it was a way to lift the skin directly without having to undergo surgery, basically a needle was put in and this thread, this suture was threaded in under the skin and pulled tight to lift the cheeks and jowls and neckline.
